The Science Behind RAMBO back on foot

The spinal cord which is the conduit of neural communication between the head and the rest of the body and when it is damaged the sensory and motor control of the portion below the damaged region is jeopardized causing loss of function and control of the activities and also the sensation depending upon the severity of the damage

Till date there have been no definitive treatment to such damage except the correction of the bony deformities to remove the compression to the spinal cord and undergo rehabilitation and some palliative measures.

The evolving specialty of regenerative medicine wherein stem cells are used as a major tool have been a great hope to this incurable condition with several reports in the literature in human patients in clinical trials proving the safety and efficacy of the autologous bone marrow stem cells.

The autologous bone marrow stem cells from Rambo thigh bone were collected

Since the quantity of stem cells in the bone marrow is usually a very small proportion, these cells were expanded (multipled) in the NCRM lab using the serum (a component of blood) of Rambo. Such advanced technology for stem celle xpansion using suitable culture cocktails is available with NCRM through their Japanese collaborators.

In the case of Rambo in addition to the simple isolation of the bone marrow stem cells we have expanded the stem cells in the laboratory and also the stem cells have been implanted into the site of the injury along with some scaffolds as this sacffolds have been proven to increase the number of stem cells after implantation.

This is the first time in a canine, such in vitro (lab) multiplied stem cells have been applied with the scaffolds with a miraculous outcome

In general, the stem cells when applied (i) secrete growth factors such as GDNF, CDNF etc which in turn indirectly promote the growth of nerve fibres to repair the damaged area to form a continuity of the injured spinal cord and (ii) undergo fusion-transdifferentiation to form neurons by themselves though the later mechanism acts in a very small proportion.

Though there have been reports earlier in animal studies about the safety and efficacy of (Ref.1-3) different biomaterials as scaffolds implanted into the site of neuronal damage, this is the first time that has been applied in a canine with miraculous outcome.

The reports have confirmed that the MAP2 and NeuN marker positive neuronal precursors are observed in vivo for a longer time than when implanted without the scaffolds(1).

When the stem cells are implanted with scaffolds, they produce more such stem cells/precursors with relatively more growth factors and neuronal cells yielding better outcome which has proven itself to be effective to this miraculous recovery of Rambo.

Thanks to the team of clinicians of TANUVAS who compassionately treated Rambo and the scientists of NCRM who with their state-of-the-art cutting edge technology using nano biomaterials have expanded the stem cells of Rambo in the lab.

Stem cell therapy is undertaken as an experimental therapy and for human applications it is governed by the guidelines issues by ICMR( Click to read )

This was done as a joint work between TANUVAS and NCRM following a MoU signed (click to read) & the earlier data of the study when started were presented in the annual symposium at NCRM NICHE in October 2009 (click to read)
